Public hospitals in the field of cardiology, geriatrics or internal medicine, and ethical experts are sought for developing personalised medicine solutions for the elderly within research cooperation agreements. Description

Cardiac failure (CF) is the most prevalent cardiovascular disease (CVD) at an international level: its prevalence is progressively growing due to the ageing population, the risks factors associations (high blood pressure, diabetes mellitus or obesity among others), social co-determinants of health and the increasing survival rates in patients with CF due to the efficacy of the evidence-based rehabilitation pathways in place.  Rehabilitation programmes and care pathways have demonstrated their efficacy for reducing the functional disability, the hospitalisations and for enhancing the quality of life of complex chronic patients. This project aims at design, implement and evaluate new care pathways for complex chronic patients with CF, merging together a novel intervention programme that has been recently validated, specific health literacy on CF and the validated CDSMP (Chronic Diseases Self-Management Programme) for improving the self-management competencies in complex elderly patients. 

The current consortium consists of hospitals and a university that will develop the evaluation framework. The project will be submitted to the call “SC1-BHC-24-2020: Healthcare interventions for the management of the elderly multimorbid patient” (maximum budget ranges between 4 and 6 M€).

The project has duration of 200 weeks approximately. Deadlines for the call and expressions of interest are 24th September 2019 and 6th September 2019, respectively.

Advantages and Innovations

Novel pathways for healthcare will be designed based on clinical complexity, frailty and functional dependency. The pilot will be implemented in collaboration with Primary Care, the Cardiology/Internal Medicine Department and the Rehabilitation Service of the hospitals that participate in the proposal.
